We accept most major insurances. It is your responsibility to make sure your services with us will be covered. Please call your insurance carrier and verify that Orthopedic Surgery and Sports Medicine and your Doctor (this is best verified by confirming the billing address) are in your network. If you have one of the following insurances, your insurance company requires referrals/authorizations to be on file in our office prior to being seen.

What is a referral?

An insurance authorization/referral is a special kind of pre-approval that individual health plan members must obtain from their chosen primary care physician before seeing a specialist.

How to request an authorization/referral from your Primary Care Physician:

  • Tell them what body part is hurting you
  • Tell them you want to schedule an appointment with the doctor you wish to see at OSSM
  • Ask them to fax a referral to our clinic(208) 684-1226.

Once we receive the insurance authorization/referral we will contact you to schedule your appointment. If you don’t hear from us after 1 week, please call back so we can follow up.

Payments

Co-payments are due at the time of the visit. If you do not have insurance we would be happy to make payment arrangements or provide a discount for payment in full at the time of the visit.
